1. Tickle Your Adrenals with this Therapeutic Breathing Exercise

“In the body, laughter serves an important purpose. It quite literally tickles the adrenals. The diaphragm is located just above the adrenal glands, which house our reactivity, our fear and anger, our apathy and hatred. When we laugh, we flex and release the diaphragm. This gives a light jiggle to the adrenals that I think of as a tickle. … In my experience, the adrenals are often quite relieved by the invitation to relax and let go.”

This is your friendly reminder to laugh and “tickle your adrenals” today. Laughter is, after all, the most therapeutic breathing exercise 😊

2. Becoming More Human, Not Superhuman

“It’s very intuitive: If we breathe better…we can deliver more oxygen to our muscles and organs, including the heart and brain, and thus heighten our physical capacity. All we’re really doing is assisting the body in working the way it was meant to work in the first place.”

That’s a great reminder that breathing (and other mind-body practices) are not hacks to boost our health to superhuman levels. They’re simply “assisting the body in working the way it was meant to work in the first place.”

We’re becoming more human, not superhuman.

3. A Mind-Blowing Study on Sleep: Is It All in Your Head?

Of course, sleep is critical to health, and if this study was performed over a long duration, its results would likely change. But this passage from Dr. Langer is an extraordinary reminder of the power of the mind:

“Our intervention was simple: We programmed a bedside clock to alter the amount of time that participants thought they had slept, irrespective of their actual sleep duration.

When the clock was sped up, such that people thought they had slept for eight hours but had slept only for five, their reaction times were quicker on an auditory psychomotor vigilance test as compared to their performance when knew they had five hours of sleep. Conversely, when people slept for eight hours but thought that they only slept for five, their performance was worse than when they had slept eight hours and thought they had slept eight hours. Clearly, our perceptions of how much we’ve slept, and not just the actual number of hours, matter.

The most common mistake in breathing is using the nose, lungs, and diaphragm, but not the heart.

P.S. I had a few nights of poorer-than-normal sleep and noticed that my morning breathing was nowhere near as enjoyable or effective. Which inspired this purposefully playful sentence : )

So the next time we need to choose an effective & appropriate response in a stressful situation, let’s first tune into our breath to “establish greater calmness in both the body and the mind,” allowing us to move forward with clarity.

Based on this description, having good working memory is pretty crucial since it plays a role in “anything you set out to do.”

But it’s relevant here because a 2022 study found that slow breathing significantly improves working memory.

Over thousands of years of evolution, fast and powerful pathways between the respiratory system and the brain have been established. If breathing stops, death occurs within minutes, and so the brain prioritizes messages from the lungs above all others.

1. How to Deal with Distraction (a method that actually works)

I realized then that to recover from our loss of attention, it is not enough to strip out our distractions. That will just create a void. We need to strip out our distractions and to replace them with sources of flow.

When we talk about focus, we hear a lot of the same advice. Put away your phone. Delete apps. It’s your own fault; you just need discipline.

However, Hari provides a better approach: Seeking out flow, I learned, is far more effective than self-punishing shame.

There are lots of ways we can do this, but my favorite is to use a focusing breathing technique to put us in a flow-like state. Then, we get to work.

Remember: Don’t shame yourself for lack of discipline. Instead, create flow.

2. Natural Resistance Breathing: Aquatic Exercise Strengthens Breathing Muscles

The lungs are receiving a greater volume of blood as well, which, combined with the pressure that water exerts on the chest wall, makes them work harder to breathe—approximately 60% harder than on land. This means that aquatic exercise can strengthen the respiratory muscles and improve their efficiency.

If you don’t have a device for resistance breathing, here’s a free alternative: aquatic exercise. You get the calming effects of the water while also strengthening your breathing muscles. A natural and soothing win-win 🙏

3. Breath Counting, Worry, and Falling Back to Sleep

Counting is handled by the same area of the brain that’s responsible for worrying. It’s difficult to do both at the same time, so counting is exceptionally effective at crowding out stress, calming a busy brain, and enhancing focus.

It’s hard to count and worry at the same time. So, when we wake up, and our minds start worrying about things, breath counting is particularly helpful.

Here’s how to use it: simply count in your head each time you exhale. Count up to 10, and then restart at one until you fall asleep. Simple & highly effective.

1. Thirty Days to Better Sleep and Higher Vagal Tone

Taken together, our results suggest that slow-paced breathing performed before sleeping may enhance restorative processes at the cardiovascular level during sleep.

Here’s another excellent study from Laborde & colleagues, with lots we could cover. But the take-home messages were that 15 min of slow breathing (6 breaths/min) before sleep for 30 days led to:

  • Significantly better subjective sleep quality

  • Significantly higher nighttime vagal tone (via HF-HRV)

  • Higher (but not significant) morning vagal tone

Not bad for just 15 minutes a night 👏

2. The Centering Breath (and 5 Breaths for Better Focus)

[B]reathe in for six seconds, hold that breath for two seconds, and then breathe out for seven seconds. When you modulate your breathing this way, you're controlling your state of arousal and corralling your body's natural response to stress.

Here’s a nice breath to try whenever you need to re-center. In for 6, hold for 2, exhale for 7. Even just one of those is enough to reset your focus.

It’s all about attention, says anesthesiologist Sam Sharar ... If we focus on a painful sensation, it will increase our experience of that pain. But if we think about something else—something safe, pleasant, far away—the pain we feel is dimmed.

Cure: A Journey into the Science of Mind over Body

Slow breathing practices have consistently been found to reduce pain. This passage helps explain how: They give us something to focus on. And more than that, they also make us feel safe, which could be even more helpful.

And although this is referring to physical pain, I think these same mechanisms help explain why it’s so beneficial for emotional distress, too.

3. Our Somewhat Unusual Breathing Patterns During Sleep

In 1984, an editorial review on breathing during sleep had conclusions that were somewhat surprising (and opposite of what I expected):

  • Our breathing rate is variable, especially during REM sleep (I anticipated it would be slow and steady)

  • Our breathing volume is reduced significantly (by as much as 16%)

  • We frequently experience hypoxia (low O2) and hypercapnia (high CO2)

I guess we could say: The more we sleep, the less we breathe, the more we heal, and the longer we live : )

1. Mindful Low-and-Slow Breathing is “Almost Always Helpful”

“Mindful low-and-slow breathing (Chapter 2) activates the parasympathetic (relaxation) nervous system and is therefore almost always helpful in reducing physiological arousal, including bringing down skin conductance. Reducing skin conductance with low-and-slow breathing is particularly helpful for a quick recovery break, between meetings, or as a break from challenging activities of the day.

Mindful low-and-slow breathing is “almost always helpful in reducing physiological arousal.” <— sounds good to me.

Use it anytime, anywhere for a quick reset and recovery.

2. Sit Up Straight: Take a Full, Deep Breath

Our thoracic curve affects lung function. When the thoracic curve become more pronounced (think hunchback), the lungs’ ability to expand decreases. Excessive thoracic curvature weakens respiratory muscles and restricts your ability to take a full, deep breath.

We probably don’t need another reminder to sit up straight. We know good posture is essential for just about everything (especially breathing; and vice-versa, as good breathing can lead to better posture).

But, for some reason, it really stuck out here and has inspired me to make my posture a top priority. Maybe this passage will spark something for you too 🙏

3. Do This when Life Gets Complicated: Two Hours instead of One

The non-negotiable part is key. When life gets complicated, these four practices are typically what we remove from our schedule, but the research shows this is the last choice we should make. When life gets complicated, lean into these practices, as they’re how you get the creativity needed to untangle the complicated.

Kotler is discussing four key practices for “sustained peak performance” (which he says are gratitude, mindfulness, exercise, and sleep).

But the idea applies to anything we do for better health (like breathing).

When life gets absurdly busy, lean into these practices. As Gandhi said, “I have so much to accomplish today that I must meditate for two hours instead of one.

4. Sleep Better with Breathing

Car accidents and heart attacks will spike today due to the time change (which is why I haven’t participated in two years—be the change you want to see style.)

2. A Revolutionary Finding: The Two-Way Street of Breathing and Emotions

Lo and behold, the participants started to feel the emotions that corresponded to the breathing patterns. In other words, when they took deep, slow breaths, the participants felt calm. And when they took rapid, shallow breaths, they felt anxious or angry. The finding that we can change how we feel by using our breath is revolutionary.” * (my emphasis)

We know that how we feel affects our breathing. However, research tells us it’s a two-way street: We can change our breathing to change how we feel.

Quite revolutionary, indeed.

3. Wim Hof vs. Wim Hof: Which of His Components is Most Important?

A 2020 study that’s still in preprints (so it hasn’t undergone peer review yet) shed light on the individual components of the Wim Hof Method (WHM).

Here’s what they did:

  • One group practiced the breathing only.

  • One group practiced cold exposure only.

  • One group practiced cold and breathing.

  • One group was trained by Wim Hof.

  • One group was trained by a researcher.

Then, all the groups were injected with endotoxin, like the first WHM study.

Here are a few of the key findings:

  • Breathing by itself reduced inflammation significantly, but did not reduce flu-like symptoms.

  • Cold by itself reduced flu-like symptoms, but did not reduce inflammation. (This doesn’t mean cold exposure doesn’t reduce inflammation in general. It just means it didn’t lessen the acute ramp-up of inflammation brought on by the endotoxin.)

  • The combination of cold and breathing reduced inflammation even more. (So, cold exposure wasn’t helpful on its own for inflammation, but added a boost when combined with the breathing.)

  • The results were the same whether Wim trained them or not (this was most surprising to me).

As a breathing nerd, this paper was super satisfying to read.

But remember it’s still in preprints, so it could turn out there was a major flaw or bad data or something. But, still fun to geek out on nonetheless : )

4. Breathing Like Journalists: Slow Breathing for Everyday Life

I call this approach, in which you fit deep work wherever you can into your schedule, the journalist philosophy. This name is a nod to the fact that journalists…are trained to shift into a writing mode on a moment’s notice

Breathe deeply and regularly…Try to breathe through your nose, keeping your mouth closed if at all possible. The more you breathe through your nose, the easier you will find it to breathe through your nose. In other words, use it or lose it.

I didn’t expect to find anything about breathing in a book called “Learn Like a Pro,” which is geared mainly toward college students. But, to learn well, you need to sleep well. Enter: the power of the breath.

And their advice for falling asleep? Inhale for 4 sec, hold for 4 sec, exhale for 6 sec, hold for 2 sec. “This type of breathing balances both the oxygen and carbon dioxide levels in your body and allows you to relax more deeply.

It’s just so awesome to see slow nasal breathing make its way into random places like this. If put into practice, these few sentences could change a student’s life forever. Simply amazing.

2. Beating Gravity with Your Nose

The results presented in this work provide evidence that the development of a substantial production of NO in the upper airways of humans may be an important part of our adaptation to life on two legs to ameliorate the influence of gravity on pulmonary blood flow distribution.

Translation: Nasal nitric oxide might have been an evolutionary adaptation to counteract gravity, allowing us to sit and walk upright. 🤯

Gravity moves blood flow toward the base of the lungs. Nitric oxide, however, redistributes blood flow help better utilize the massive surface area of the lungs. This allows us to get more oxygen in the upright position.

So go take a walk, breathe through your nose, oxygenate your body, and enjoy this gift evolution has given us.

Inhalation causes an immediate rise in heart rate, followed (∼5 s) by increased blood pressure and baroreceptor firing. Exhalation results in an immediate decrease in heart rate followed (∼5 s) by decreased blood pressure and baroreceptor firing.

Have you watched the Huberman Lab video showing how breathing immediately impacts heart rate? Check it out. The above passage explains how that process goes on to improve HRV.

Specifically, it’s that ~5-second lag between the rise and heart rate and rise in blood pressure that’s critical. And it’s this lag that makes breathing at a 5in/5out rhythm so beneficial (although it’s slightly different for everyone).

When we breathe like this, the messages from our breath and blood pressure synchronize, increasing their amplitude and increasing HRV.

Thus, there’s to magic behind how slow breathing improves HRV. It’s simply a harmony of body messages, which increases efficiency and, subsequently, improves resiliency and overall health.

2. Breathing’s Version of Powered In, Unplugged, and System Restarts

  • Nasal breathing is like having your computer plugged in. You’re getting a constant supply of energy via your power chord (nose and nasal airways).

  • Mouth breathing is like unplugging from the charger. Sometimes it’s needed, and you can make it for some time, but eventually you’ll run out of juice.

But suppose you keep your computer plugged in 24/7, even at night when you close your laptop (via mouth tape).

What happens in that case? Well, even then, you’ll eventually have too many things running. You’ll need a restart.

Methods like Wim Hof/SKY/Tummo serve as this restart for your nervous system. They clean out all the junk, allowing you to return to your baseline.

Here’s to using our breathing to optimize our energy and meet whatever demands our systems have.

3. “Can Breathwork Help You Sleep? An Expert Explains”

If you want to engage in breathwork for sleep, don’t get too hung up on the details. The key is to slow down your breath and really direct it to your belly using your diaphragm.

In this article, Molly Atwood, an assistant professor of psychiatry and behavioral sciences at Johns Hopkins University, explains why deep breathing is so helpful for sleep. It’s super quick and practical—I loved it.

It was especially refreshing that there was nothing fancy or complex: “It’s not a super complicated thing to practice,” she says. “I think it would be hard to find something that would steer you completely wrong.” Amen, and enjoy!

This knowledge is spreading back to the West through disciplines such as yoga and mindfulness, but also through techniques aimed at improving endurance, and even intimacy. These practices demonstrate that the mind and the heart follow the lungs, not the other way around.

Study after study has shown that breathing gives you access and control over your heart and mind, in ways such as increasing heart rate variability and synchronizing brain waves. That is, the lungs lead, the heart and mind follow.

But even with all this research, sometimes it takes an eloquent quote from a respected pulmonologist to make it seem so obvious.

2. Why Slow Nasal Breathing Could Be More Important in Type-2 Diabetes

Both type-1 and type-2 diabetes benefit from slow nasal breathing.

However, it could be more beneficial in type-2 diabetes since the primary issues there are reduced insulin production and reduced insulin sensitivity.

And it just so happens that slow breathing increases insulin production and improves insulin sensitivity. But maybe most importantly, nasal breathing helps you sleep better, which will indirectly boost insulin sensitivity.

Obviously, slow breathing isn’t going to cure you of type-2. But it’s safe, effective, and super practical. It seems like a no-brainer.

P.S. For T1Ds like me, all these things are still very helpful. We just won’t get the added benefit of increased insulin production…stupid pancreas…

I’ve become somewhat obsessed with breathwalking. I use it in short 1-3 min intervals several times a day. It’s phenomenal. Thanks again, Louise!

Last Tuesday, I was walking around my backyard, completely focused on my breath. Then, I came about this close 🤏 to stepping on a snake. The snake had its head up, ready to bite. And here I am, Mr. Breathwalker, completely oblivious…lol.

Luckily, I managed to jump over it (maybe letting out a quick scream) and immediately starting laughing at the irony of the situation.

Focusing on your breath most certainly improves your concentration. But in the comfort of your backyard, that concentration might backfire!

The Lesser-Known Benefits of Nasal Breathing, Designed for Diabetes

 

As a person with type 1 diabetes, my experience with nasal breathing has been nothing short of miraculous.  It's been such a simple change, yet its impacts on my energy and blood sugars have been profound. I feel it would be irresponsible not to share it with other people with diabetes.

Luckily, people much brighter than me have become fascinated by the nose too.  Whole books have now been written on the topic.  It has been featured in popular sources such as Outside Magazine and the Cleveland Clinic.  An article published in Frontiers in Human Neuroscience even concluded that “nasal stimulation represents the fundamental link between slow breathing techniques, brain and autonomic activities and psychological/behavioral outputs.”

But after several years of research, I have come to realize that the reasons nose breathing is so helpful for diabetes go far beyond the “obvious ones.” 

Of course, the usual suspects are essential, such as the warming and humidifying of the incoming air and the natural slowing of the breath.  But to fully understand the benefits for diabetes requires a synthesis of research from different fields, such as diabetic complications, the metabolites of nitric oxide, chronic stress, and sleep.

Let’s start with diabetic complications.

Oxidative Stress and Inflammation Reduce Blood Flow

Over time, the oxidative stress and chronic inflammation associated with diabetes can damage blood vessels, resulting in poor circulation

Less blood flow means that less oxygen reaches the cells, tissues, and organs

As a result of this poor circulation (and other complications), people with diabetes have an increased incidence of retinopathy, kidney disease, and foot problems.

Nose breathing—specifically inhaling through the nose—immediately helps with this.  For example, one small study showed that just five minutes of inhaling through the nose and exhaling through the mouth increased tissue oxygenation by 10%.  This increase was due to nitric oxide.

Nitric oxide is continuously produced in the paranasal sinuses.  When you breathe in through your nose, nitric oxide is carried into the lungs, where it opens up the blood vessels and improves blood flow in the lungs.  This results in better gas exchange and better blood oxygenation.

But that’s only the beginning of NO’s benefits.

How Nitric Oxide Helps with Blood Flow and Oxygenation

Typically, the nitric oxide produced in your nose is treated separately from the nitric oxide produced throughout the rest of your body.  Although it is known that that inhaling nitric oxide has effects outside of the lungs, scientists have not known how. Researchers at the Cleveland Clinic have recently shed light on the issue.

In a 2019 study, they did something simple yet meaningful.  The researchers had participants inhale extra nitric oxide, and they measured what happened in the blood afterward.  If nitric oxide’s journey ended in the lungs, they wouldn’t see any signs of it in distant blood samples.

The results showed the opposite

They found that inhaling nitric oxide significantly increased circulating levels of a specific form of the molecule, SNO-Hb.  These findings matter because, in a separate study published in PNAS in 2015, a different group of researchers found that SNO-Hb played an essential role in whole-body oxygenation.  Without it, mice received less blood flow to the heart and even had smaller litter sizes.

Why This is Important to Diabetes

The complications of diabetes also impact nitric oxide.  Sustained high blood sugars alter how hemoglobin stores nitric oxide. 

The end result is that people with diabetes generally have less SNO-Hb

And, as we just learned, SNO-Hb is critical to blood flow and tissue oxygenation. 

So, putting it all together:

  • People with diabetes suffer from poor circulation and insufficient oxygen.

  • Our noses are a source of nitric oxide—breathing through our nose utilizes it.

  • Inhaling nitric oxide increases an essential form of NO called SNO-Hb.

  • SNO-Hb is critical to improving blood flow and increasing whole-body oxygenation.

Therefore, nose breathing could be especially helpful in diabetes by maintaining normal SNO-Hb levels and hence helping improve blood flow and oxygenation throughout the body.*  Mouth breathing would not provide these benefits.

How High Blood Sugars Reduce Oxygen Delivery

In addition to altering how nitric oxide is stored, high blood sugar also modifies the relationship between oxygen and hemoglobin.  Specifically, it tightens the bond between them.

As a result, less oxygen can be delivered to the places it is needed.  (For the breathing nerds out there, it causes a left shift in the oxyhemoglobin dissociation curve.)  This problem might be further exacerbated by stress.

Chronic Stress and Carbon Dioxide Alter Oxygen Availability

Diabetes also causes chronic stress—a less intense but sustained fight or flight stress response.  This stress causes people with diabetes to have anywhere from 14% to 20% more cases of anxiety than those without it.  Moreover, it has been reported that up to 40% of the diabetic population show symptoms of anxiety.  A 2013 meta-analysis, including over 12,000 people with diabetes, also found significant associations between diabetes and an increased probability of anxiety disorder or anxiety symptoms.

Chronic stress and anxiety, such as that experienced in diabetes, are often associated with overbreathing.  Overbreathing, or hyperventilation, simply refers to breathing more than your metabolic demands at any given moment, and is often associated with mouth breathing. Consequently, the body gets rid of too much carbon dioxide, which alters the pH of the blood.

This has a similar effect on oxygen and hemoglobin as high blood sugar. 

That is, it tightens the bond between them, reducing the amount of oxygen that can be delivered to the cells and tissues (this is known as the Bohr effect).

Together, we see that the high blood sugar and chronic stress associated with diabetes combine to reduce oxygen availability to the cells and tissues.

When we switch to nose breathing, the volume of each breath is naturally reduced.  This helps normalize carbon dioxide levels and restore blood pH to normal levels, which will improve oxygen delivery.** 

Combining this with the earlier discussion on nitric oxide, we see how this one simple change (nose breathing) helps offset some diabetic complications.

Nose Breathing, Sleep, and Diabetes: The Missing Link to Better Blood Sugar Control

If we breathe nasally during sleep, all of the benefits of nose breathing continue throughout the night. Of all the things nose breathing helps with, this might be the most critical for diabetes.

We have already discussed that diabetes causes chronic stress, which can lead to more rapid breathing.  Nose breathing helps naturally slow down the breath.  This will help you shift from a stressful sympathetic state to a calming parasympathetic state.  This shift is significant for people with diabetes who exhibit less parasympathetic tone at night than non-diabetics.  Thus, nasal breathing at night helps us increase parasympathetic tone and enjoy better sleep.

Receptors in your nose also act to maintain rhythmic breathing during sleep.  This might help explain why nose breathing reduces the risk of obstructive sleep apnea when compared to mouth breathing.  Diabetes is associated with a significantly increased risk of obstructive sleep apnea. So, if nasal breathing at night can help reduce this risk, it could be especially beneficial.

Lastly, we know that inadequate sleep causes insulin resistance.  By getting deeper, more restorative sleep, insulin sensitivity can be improved.  This could potentially lead to better morning blood sugars (that was my experience), setting you up for a better day of glucose control.

The More Subjective but Most Important Benefit of Nasal Breathing for Diabetes

Altogether, nasal breathing increases blood flow, improves tissue oxygenation, and appears to increase an essential form of bioactive nitric oxide that people with diabetes have less of.  It also improves sleep quality by helping us flip to a more calming state and by reducing the incidence of sleep apnea.  This can help improve insulin sensitivity.

When we combine all of these together, nasal breathing's net benefit can be simply stated as:

It gives you more energy. 

And it’s harder to objectively measure, but perhaps the best aspect of increased energy levels is more motivation to take care of your blood sugars. 

Diabetes is an exhausting full-time job that can lead to physical and emotional burnout.  Having more energy and enthusiasm to manage the disease could be the most valuable aspect of something as simple as breathing through your nose.

*The levels of NO inhaled in the 2019 Cleveland Clinic study were greater than those produced in the nose.  But, the study validated that there is a mechanism by which NO that is introduced into the lungs can be transported throughout the body as SNO-Hb.  Thus, nasal breathing would only act to bring SNO-Hb up to normal physiological levels, whereas mouth breathing would rob the body of this important physiological process.

As I have been making my way through Patrick McKeown’s latest book, The Breathing Cure, I’ve been reminded of just how powerful all this breathing stuff is. Patrick has sections on breathing for diabetes, epilepsy, hypertension, and more.

Of course, despite the catchy title, breathing does not "cure" any of these ailments. Still, it’s quite remarkable to see all the conditions it can help with.

Upon reflection, this makes a lot of sense if we remember that breathing is like an index fund for health. It invests a small amount into a wide range of the body’s functions, such as lung and heart health, autonomic balance, and sleep.

Over time, these tiny improvements combine and compound into overall better health. And as Patrick’s new book shows, these benefits can be helpful in many different conditions.

The key, however, is to find a breathing practice that is right for you, get started, and be consistent. Here’s to safely investing in your health today.

P.S. Unlike most financial investments, however, you also get an immediate return on investment with breathing. For example, just two minutes of slow breathing can improve autonomic balance and enhance decision-making.

Becoming a world-class breather is actually quite simple: reduce the complexity, optimize the simplicity.

Of course, you can still utilize a variety of advanced techniques. Kobe Bryant didn’t only practice shooting free throws, and Tom Brady doesn’t only practice taking snaps. But it all starts with optimizing the fundamentals.

The best way to do that? Nose, belly, quiet, slow, repeat. Let those benefits compound into "insane revolutions over time."

2. What Optimal Breathing Can Do for Diabetes 

Although it’s not a panacea, improving our breathing might be the simplest thing we can do for our overall health. And for diabetes, in particular, optimal breathing has several direct and indirect benefits that are especially useful.

For example, it can improve cardio-autonomic function, reduce stress and anxiety (see next thought), improve blood flow, and improve sleep. These benefits can lead to better insulin sensitivity, more stable blood sugars, and less risk of long-term complications. Not bad for something as simple as breathing.

Most skills you learn in tennis don’t make you better at hockey.

But the skill of breathing is universal, applicable in every domain, available every second of every day.

2. A New Foundation for the Pillars of Health

Beautifully said, but I would go a step further and state that breathing is the bedrock of healthy sleep. Remove correct (nasal) breathing at night, and even excellent sleep hygiene becomes less effective.

Of course, breathing isn’t a cure-all that replaces the other health pillars. But, it is the foundation on which they all sit. We’d be wise to make it a solid one.

For any situation you’re in, you can find a cliché to help you through. And as seen above, clichés are often contradictory. Likewise, breathing clichés are also contradictory, as we saw about a month ago:

Wim Hof tells you to breathe more. // Patrick McKeown tells you to breathe less.

Lung capacity determines longevity. // But you also shouldn’t take big breaths.

Oxygen is your body’s most important energy source. // But the exhale is the most important part of the breath.

For breathing, I suggested that we embrace these contradictions. But from a practical perspective, I like Neil Pasricha’s advice: "Any cliché, quote, or piece of advice that resonates with you only confirms to your mind something you already know."

We’re all different. So if you notice a specific breathing method standing out, it’s probably confirming something you already know to be right for you.

2. Stop Breathing Sugar

"Cutting sugar-sweetened beverages from your diet is the single-biggest thing you can do to improve your health."

Many different diets work for many different people. But they all pretty much agree on one thing: remove processed sugar. And Mark Hyman’s number one recommendation is even more straightforward: "Don’t drink sugar."

Similarly, despite all the inconsistencies in breathing advice, there is one thing we can all agree on: don’t breathe through your mouth. Mouth breathing is like drinking sugar. It’s easy and it feels good, but it is detrimental to your health.

Let’s cut this processed "breathing sugar" from our diets. It may very well be the most important thing we can do for our health.

3. Nitric Oxide Reduces Alveolar Dead Space

"Alveolar dead space…represents alveoli, typically in the apex of the lung in an upright person, that do not receive blood flow"

By redistributing blood flow in the lungs, inhaled nasal nitric oxide reduces the alveolar dead space described above. Specifically, it allows more alveoli to receive blood flow, which allows more gas exchange to occur. This is something you won’t find in textbooks because the science is just too new.

This remarkable effect has led some to postulate that nasal nitric oxide is an evolutionary adaptation that helped allow us to walk upright. Pretty neat.

4. Slow Breathing is Better Than Social Media for Sleep

"Slow-paced breathing appears a promising cost-effective technique to improve subjective sleep quality and cardiovascular function during sleep in young healthy individuals."

- Journal of Clinical Medicine (2019)

This study found that 15 minutes of slow breathing (6 breaths/min) before bed led to better subjective sleep quality than 15 minutes of social media use. Slow breathing also led to higher overnight vagal activity.

Although these results might seem like common sense, I thought they were still compelling nonetheless. Here's to putting away our phones and focusing on our breath before bed.

1. A New Breathing Gas Discovered

"Thus, H2S [hydrogen sulfide] produced by CBS surely has a crucial role in maintaining eupneic-pattern respiration."

Hydrogen sulfide. It’s toxic, and it’s what gives rotten eggs their rotten smell. It also helps maintain our breathing.

A study published in Nature: Communications Biology on October 16, 2020, found that a small amount of hydrogen sulfide is critical to maintaining normal breathing. It does this through modification of the neural connections in the region of the brain that controls breathing.

When hydrogen sulfide production was stopped in rats, normal breathing turned to gasping, implying that it is vital to maintaining rhythmic breathing.

From a practical perspective, I’m not sure what this means yet. At a minimum, breathing now appears to be a four-gas system: oxygen, carbon dioxide, nitric oxide, and hydrogen sulfide.

We’ve found ways to breathe to optimize the first three. However, it’s not yet clear if the way we breathe (or do anything else, for that matter) can influence hydrogen sulfide.

In any case, as a fellow breathing nerd, I hope you find this as fascinating and humbling as I did. Here’s to the joy of never-ending learning.

2. The Single Most Important Part of the Breath

"Interestingly, Törnberg et al. recently showed that more NO is released from the nasal passages during nasal inhalation compared with exhalation." - Lundberg et al. (2008)

Here is yet another reason why inhaling through your nose might be the single most important part of breathing: nitric oxide (NO) production is much greater during inhalation than exhalation.

Here’s the finding from the paper mentioned above: "There was a marked flow dependency of nasal NO output, with the highest levels observed during inhalation at flows similar to those seen during normal breathing." - Törnberg et al. (2002)

In hindsight, this makes complete sense. NO helps protect our lungs from inhaled pathogens and it redistributes blood flow in the lungs. So, of course our bodies would maximize it on the inhale. But it wasn’t until I read these two papers that it made sense to me.

Action is the cornerstone of all improvement. We can read and learn all we want, but that is only storing potential energy. Action converts that energy into something useful.

Breathing is the most primitive form of taking action. It gives you something you can do, that actually does something. Breathing induces physiological and neurological changes in your state that are truly useful in any real-life situation.

It’s no wonder all ancient traditions focused on the breath. Before the internet, before you could major in positive psychology, before life coaches and Harvard studies, there was the breath. Breathing is the ultimate self-improvement tool.

2. Insomnia Identified as New Risk Factor for Type-2 Diabetes

I guess this shouldn’t be surprising, given that even one night of sleep deprivation significantly increases insulin resistance. But is there anything we can do about it? You’ve probably guessed my answer by now : )

Of course, "breathing" isn’t the cure for everything, and it certainly isn’t a magic pill for insomnia. But it might help.

Slow breathing activates the parasympathetic nervous system, relaxing the body, preparing it for sleep. Once asleep, nose breathing helps you wake up less (see last week’s 411), maintain rhythmic breathing, and ultimately sleep deeper. All of this might help reduce stress hormones and increase insulin sensitivity.

Overall, a simple change to your breathing, compounded over time, might help reduce your risk of type-2 diabetes (or at least help you manage it better), even if only the tiniest little bit.

We discuss carbon dioxide tolerance a lot. But I prefer James’ terminology, using chemoreceptor flexibility rather than CO2 tolerance. Flexibility implies variability. It also implies robustness.

Of course, I believe the most critical part of this flexibility is the ability to withstand higher CO2, that is, CO2 tolerance. But let’s not forget about robustness and adaptability. Tension and relaxation. Stretching in both directions, not just one.

We’ve discussed how inhaled oxygen can lead to adverse effects in people with diabetes. Specifically, high levels of inhaled oxygen can reduce arterial function. But there might be other problems with oxygen therapy, especially when done over prolonged periods.

For example, your lungs have their own microbiome (pretty neat). And a recent study showed that inhaled oxygen harms this microbiome, leading to an increased risk of lung damage. This idea is especially relevant during COVID-19:

3. David Blaine on Breath Holds, CO2, and 45 Minutes Without Air

David Blaine was recently on the Joe Rogan Podcast. They began talking about breath-holds within 5 minutes (this link should take you right to it).

Here are two quotes I loved from it:

"The breath-holding thing is all about like, a CO2 build up in the bloodstream, and it’s about a tolerance level to it…"

And when Joe asks about the panicked feeling you get when holding your breath, David’s reply is:

That’s not an O2 deprivation. That’s a trigger from a CO2 build-up, which is giving you an alert…"

All of you "breathing nerds" already knew this. But it is little tidbits like these that help the general population learn.

He goes on to talk a little about pre-breath-hold hyperventilation, blacking out, and how we can go 45 minutes without air.

4. Is there a Perfect Breath?
