1. Science Talk for “Breath is Life”

“We must continuously breathe precisely to bring oxygen to mitochondria to fuel oxidative phosphorylation, to charge our mitochondria, which sustain cellular life, brain activity, and consciousness.”

That’s a scientific way of saying: Breath is life.

So, let’s use this passage as inspiration to occasionally take a mindful breath, remembering that each one sustains “cellular life, brain activity, and consciousness.”

2. Stress, Inflammation, and a Valuable Tool for Coaches

“Stress-reduction techniques may serve as cost-effective interventions for preventing and treating metabolic disease.”

– Jurgens et al. (2023), Brain, Behavior, & Immunity

The results of this paper suggest that stress-lowering exercises (like slow breathing) may reduce inflammation and, consequently, improve metabolic function. 👏

That sounds a lot like a breath practice. You breathe a specific way, have a clear goal, and a way of achieving it: your breath. “The simplicity may be the secret to finding inner-direction.”

The best part? You don’t even have to leave your house 😊

2. Becoming Students of Life and Breath

“Life is always trying to show us something. It’s communicating with us through the events, people, and ideas that show up in our lives.”

It’s also communicating with us through our breath. When we tune into our breath, we tune into the signals life is sharing, giving us the opportunity to cultivate awareness and make changes if necessary.

So here’s to frequently aligning with our breath so we can be better students of life, today 🙏

3. How Slow Breathing May Help Inflammation and Metabolic Disease

“Our findings suggest that stress can contribute to a state of chronic, low-grade inflammation that leads to metabolic dysregulation. … Stress-reduction techniques may serve as cost-effective interventions for preventing and treating metabolic disease.”

If anything is (almost) certain about controlled slow breathing, it’s that it helps reduce stress. That’s why this study is so important.

It showed that stress is associated with metabolic dysregulation through inflammation. Association doesn’t mean causation, but this provides a pathway for slow breathing to help these conditions.

Namely, by reducing stress, slow breathing may lower inflammation and thus reduce our risks of metabolic dysregulation 👏

4. Spiraling Into Control

When life feels chaotic, slow nasal breathing helps you and your nervous system spiral into control, for a change.

I try to keep these short and sweet, so this couldn’t make it as a thought. But it’s SO GOOD, so I wanted to share it somehow.

A Powerful and Genuinely Life-Changing Thought Experiment

If they’re golfers, I sometimes suggest to them that they imagine that God appeared to them and said, ‘You’re going to have a great career. You’re going to win dozens of tournaments. 1. Breathing Exercises Lower Inflammation and Change Gene Expression

In those who practice breathing exercises, levels of inflammatory proteins in the blood are significantly lower, especially under certain types of stress. Mobilizing the power of the breath has also been shown to turn on anti-inflammatory genes and turn off pro-inflammatory ones, including genes that regulate energy metabolism, insulin secretion, and even the part of our DNA that controls longevity.

2. These Two Simple Rules Will Improve Your Breathing Forever

If I could give anyone any advice for everyday breathing, it’d be this:

  1. Breathe through your nose, especially during sleep.

  2. Make your breathing quiet.

You’ll get massive benefits with minimal effort using these two simple rules.

It’s that simple. And the word or phrase? Dr. Benson says, “The choice of a focused repetition is up to the individual. If you’re a religious person, you can choose a prayer. If you’re a non-religious person, choose a secular focus.”*

I’ve been using it this week in my slow breathing practice. I inhale and then repeat “be the change” in my head while exhaling at a 5 breaths/min pace.

I’ve never been a big fan of mantras (and I’m still testing different ones), but I’ve genuinely enjoyed this. It’s straightforward and highly effective.

Give it a shot and see how you feel.

4. My Fake Words, and Breathing as the Anti-Harley-Davidson

Orfield Laboratories, run by Steven Orfield, is a small Twin Cities business that leverages the power of perception to help companies build better products. Harley-Davidson, for example, once hired Orfield to calculate the exact engine tone and decibel level that would give riders the impression that its motorcycles are powerful.”*

How crazy is that!? Harley-Davidson actually hired a company to ensure their engines gave “riders the impression that its motorcycles are powerful.

I think breathing is the anti-Harley-Davidson. You just relax and breathe. If it’s powerful, it’s powerful. If it’s not, it’s not. No fake impressions needed.

2. A Revolutionary Finding: The Two-Way Street of Breathing and Emotions

Lo and behold, the participants started to feel the emotions that corresponded to the breathing patterns. In other words, when they took deep, slow breaths, the participants felt calm. And when they took rapid, shallow breaths, they felt anxious or angry. The finding that we can change how we feel by using our breath is revolutionary.” * (my emphasis)

We know that how we feel affects our breathing. However, research tells us it’s a two-way street: We can change our breathing to change how we feel.

Quite revolutionary, indeed.

3. Wim Hof vs. Wim Hof: Which of His Components is Most Important?

A 2020 study that’s still in preprints (so it hasn’t undergone peer review yet) shed light on the individual components of the Wim Hof Method (WHM).

Here’s what they did:

  • One group practiced the breathing only.

  • One group practiced cold exposure only.

  • One group practiced cold and breathing.

  • One group was trained by Wim Hof.

  • One group was trained by a researcher.

Then, all the groups were injected with endotoxin, like the first WHM study.

Here are a few of the key findings:

  • Breathing by itself reduced inflammation significantly, but did not reduce flu-like symptoms.

  • Cold by itself reduced flu-like symptoms, but did not reduce inflammation. (This doesn’t mean cold exposure doesn’t reduce inflammation in general. It just means it didn’t lessen the acute ramp-up of inflammation brought on by the endotoxin.)

  • The combination of cold and breathing reduced inflammation even more. (So, cold exposure wasn’t helpful on its own for inflammation, but added a boost when combined with the breathing.)

  • The results were the same whether Wim trained them or not (this was most surprising to me).

As a breathing nerd, this paper was super satisfying to read.

But remember it’s still in preprints, so it could turn out there was a major flaw or bad data or something. But, still fun to geek out on nonetheless : )

4. Breathing Like Journalists: Slow Breathing for Everyday Life

I call this approach, in which you fit deep work wherever you can into your schedule, the journalist philosophy. This name is a nod to the fact that journalists…are trained to shift into a writing mode on a moment’s notice

Let’s adopt this journalist philosophy to our slow deep breathing practice, fitting it into our schedules wherever we can. This will allow us to flip the switch on our physiological and emotional state anytime, anywhere (see thoughts 1 & 2).

When researchers at Harvard, Yale, and the Massachusetts Institute of Technology scanned the brains of experienced meditators, they discovered increased thickness in regions of the brain's cortex, or grey matter, related to focus and attention…But what's intriguing about this study is that breathing rate was used to determine how deep in meditation the subjects were able to get. The slower the breathing rate, the deeper in meditation participants became. And, the more pronounced their increase in grey matter.” *

That deserves a 🤯

If slower breathing rates are a byproduct (and measure) of deeper meditation, I wonder if we can reverse engineer it…

2. The Deep Breath Hypothesis

The Deep Work Hypothesis: The ability to perform deep work is becoming increasingly rare at exactly the same time it is becoming increasingly valuable in our economy. As a consequence, the few who cultivate this skill, and then make it the core of their working life, will thrive.

And similarly, this couldn’t be any truer:

The Deep Breath Hypothesis: The ability to perform deep breathing is becoming increasingly rare at precisely the same time it is becoming increasingly valuable to our physiology. Consequently, the few who cultivate this skill, and then make it the core of their waking life, will thrive.

3. Wim Hof vs. Slow Breathing, Part 2: The Famous Endotoxin Challenge

I’ve called Wim Hof’s endotoxin study the “4-minute mile” of breathing.

So, you can only imagine my excitement when I found a study basically doing the exact same thing, but with slow breathing (it was perhaps the most excited I’ve ever been reading a paper, lol).

I produced an in-depth comparison of the studies here.

But, here are some of the take-home messages:

  • The WHM reduced fever and all other flu-like symptoms.

  • Slow breathing reduced headaches and eye sensitivity to light, but did not reduce fever or other flu-like symptoms (e.g., nausea and chills).

  • Slow breathing improved autonomic functioning as measured by HRV, suggesting participants had greater resiliency. HRV was not measured during the WHM experiment.

  • The WHM significantly reduced inflammatory cytokines. Slow breathing did not.

  • The WHM significantly increased anti-inflammatory cytokines (by up to 194%). Slow breathing did not.

There are many (many) caveats and differences between the studies, so if you’re interested, see the full write-up for more details and additional thoughts.

But based on these results, the WHM was decidedly more effective than slow breathing at reducing acute inflammation and fighting off flu-like symptoms.

And I would argue that we humans are most human when we’re improving ourselves. We, unlike any other animal, can consciously change ourselves, to improve ourselves in ways we choose. This distinguishes us from every other species alive today and, as far as we know, from every other species that has ever lived.” *

We are the only species that can consciously change ourselves to improve in ways we choose. We’re also the only ones that can consciously change our breathing in ways we choose.

Starting something meaningful this year? Or just sending an important email or text? Roll up those metaphorical sleeves by taking a deep and relaxed breath in, and a relaxed and slightly longer breath out. “Let’s do this thing.

You don’t have to build the habits everyone tells you to build. Choose the habit that best suits you, not the one that is most popular.

3. Wim Hof “vs.” Slow Breathing, Part I: Indirect Comparison for Chronic Inflammation

A 2019 study looked at the full Wim Hof Method (breathing, cold, and meditation) in people with axial spondyloarthritis. They found that it reduced chronic inflammation, concluding:

…our results are indicative that voluntary modulation of the immune response may not only be possible in acute inflammatory response due to microbial stimulation but also in chronic inflammation related to immune-mediated inflammatory diseases.

A 2021 systematic review examined HRVB (aka slow breathing) in people with various chronic illnesses. They found evidence that there is an inverse relationship between inflammation and vagal nerve activity:

“…an increase in efferent vagal activity could suppress pro-inflammatory factors suggesting possible anti-inflammatory effects of HRVB.

The point? Two wildly different approaches, two somewhat similar outcomes. It brings to mind the Chinese proverb, “There are many paths to the top of the mountain, but the view is always the same.” Choose which path is right for you.

P.S. I put quotations around “vs” because there’s no competition in breathing 😊Also, the word “indirect” in the title is critical. I’m being simplistic here and comparing apples to oranges, but I think the overall message is still helpful.

That’s Pressfield’s definition of “operationalize,” and it’s exactly what we need to do with our breathing. Make it ready for live action—for real life.

Of course, let’s use our practice as an escape into calmness, but let’s also remember to operationalize it so it’s useful in our everyday lives.

Ujjayi pranayama, which is performed by lightly tightening the glottis and breathing with a whispering sound and then exhaling out of either the left or right nostril, stimulates the vagus through massaging the larynx and downregulating the sympathetic nervous system. … Ocean breath is sometimes used synonymously with ujjayi, but the two are separate practices.

I thought Ujjayi and Ocean breathing were the same thing. So, I found this passage fascinating (especially that Ujjayi uses an exhale through one nostril). I messaged Eddie about it, and he kindly responded with even more value.

He added that, for Ocean breathing, “the sound is made not so much by the tightening of the glottis, but by creating a continuum of pressure from the high nasal cavity, through the throat, into the thorax.

That means there are two key differences between Ujjayi and Ocean breathing:

  • Ujjayi: sound comes from the glottis, and the exhale is through only one nostril.

  • Ocean: sound comes from the nose to thorax, and the exhale is through both nostrils.

2. Slow Breathing for Oxidative Stress and Inflammation

Diabetes and its associated blood sugar fluctuations lead to chronic oxidative stress and inflammation. What can slow deep breathing do?

  • JACM (2011):Diaphragmatic breathing, likely through the activation of the parasympathetic nervous system…reduces reactive oxygen species production.

  • Nature (2017):…our results lead to the hypothesis that slow breathing may exert some antioxidant effect, possibly via parasympathetic stimulation.

  • PLOS One (2013):RR [relaxation response] practice…reduced expression of genes linked to inflammatory response and stress-related pathways.

Taken together, these results suggest that slow breathing could be a simple and effective way to reduce oxidative stress and inflammation in diabetes. 

But you don’t need diabetes to benefit. These two complications are present in many acute and chronic conditions, so the above results will help everyone.

4. Why You Should Start Teaching “Brooklyn” Yoga

One new meditator kept coming to interviews with a chronic lament: ‘The breath is so boring!’

Finally, I asked him if he'd ever heard of Brooklyn yoga? He said no. I told him to close his mouth tight and close off both nostrils with his fingers. We sat that way for some time until finally he let go of his nose and gasped for air.

I must have listened to this story about 10 times in a row and laughed more with each one. It brilliantly captures the importance of breathing—no science or technical jargon needed.

So if anyone tells you breathing is boring, doesn’t work, or is pseudo-science, teach them a little “Brooklyn” yoga—you might just change their mind : )

The Lesser-Known Benefits of Nasal Breathing, Designed for Diabetes

 

As a person with type 1 diabetes, my experience with nasal breathing has been nothing short of miraculous.  It's been such a simple change, yet its impacts on my energy and blood sugars have been profound. I feel it would be irresponsible not to share it with other people with diabetes.

Luckily, people much brighter than me have become fascinated by the nose too.  Whole books have now been written on the topic.  It has been featured in popular sources such as Outside Magazine and the Cleveland Clinic.  An article published in Frontiers in Human Neuroscience even concluded that “nasal stimulation represents the fundamental link between slow breathing techniques, brain and autonomic activities and psychological/behavioral outputs.”

But after several years of research, I have come to realize that the reasons nose breathing is so helpful for diabetes go far beyond the “obvious ones.” 

Of course, the usual suspects are essential, such as the warming and humidifying of the incoming air and the natural slowing of the breath.  But to fully understand the benefits for diabetes requires a synthesis of research from different fields, such as diabetic complications, the metabolites of nitric oxide, chronic stress, and sleep.

Let’s start with diabetic complications.

Oxidative Stress and Inflammation Reduce Blood Flow

Over time, the oxidative stress and chronic inflammation associated with diabetes can damage blood vessels, resulting in poor circulation

Less blood flow means that less oxygen reaches the cells, tissues, and organs

As a result of this poor circulation (and other complications), people with diabetes have an increased incidence of retinopathy, kidney disease, and foot problems.

Nose breathing—specifically inhaling through the nose—immediately helps with this.  For example, one small study showed that just five minutes of inhaling through the nose and exhaling through the mouth increased tissue oxygenation by 10%.  This increase was due to nitric oxide.

Nitric oxide is continuously produced in the paranasal sinuses.  When you breathe in through your nose, nitric oxide is carried into the lungs, where it opens up the blood vessels and improves blood flow in the lungs.  This results in better gas exchange and better blood oxygenation.

But that’s only the beginning of NO’s benefits.

How Nitric Oxide Helps with Blood Flow and Oxygenation

Typically, the nitric oxide produced in your nose is treated separately from the nitric oxide produced throughout the rest of your body.  Although it is known that that inhaling nitric oxide has effects outside of the lungs, scientists have not known how. Researchers at the Cleveland Clinic have recently shed light on the issue.

In a 2019 study, they did something simple yet meaningful.  The researchers had participants inhale extra nitric oxide, and they measured what happened in the blood afterward.  If nitric oxide’s journey ended in the lungs, they wouldn’t see any signs of it in distant blood samples.

The results showed the opposite

They found that inhaling nitric oxide significantly increased circulating levels of a specific form of the molecule, SNO-Hb.  These findings matter because, in a separate study published in PNAS in 2015, a different group of researchers found that SNO-Hb played an essential role in whole-body oxygenation.  Without it, mice received less blood flow to the heart and even had smaller litter sizes.

Why This is Important to Diabetes

The complications of diabetes also impact nitric oxide.  Sustained high blood sugars alter how hemoglobin stores nitric oxide. 

The end result is that people with diabetes generally have less SNO-Hb

And, as we just learned, SNO-Hb is critical to blood flow and tissue oxygenation. 

So, putting it all together:

  • People with diabetes suffer from poor circulation and insufficient oxygen.

  • Our noses are a source of nitric oxide—breathing through our nose utilizes it.

  • Inhaling nitric oxide increases an essential form of NO called SNO-Hb.

  • SNO-Hb is critical to improving blood flow and increasing whole-body oxygenation.

Therefore, nose breathing could be especially helpful in diabetes by maintaining normal SNO-Hb levels and hence helping improve blood flow and oxygenation throughout the body.*  Mouth breathing would not provide these benefits.

How High Blood Sugars Reduce Oxygen Delivery

In addition to altering how nitric oxide is stored, high blood sugar also modifies the relationship between oxygen and hemoglobin.  Specifically, it tightens the bond between them.

As a result, less oxygen can be delivered to the places it is needed.  (For the breathing nerds out there, it causes a left shift in the oxyhemoglobin dissociation curve.)  This problem might be further exacerbated by stress.

Chronic Stress and Carbon Dioxide Alter Oxygen Availability

Diabetes also causes chronic stress—a less intense but sustained fight or flight stress response.  This stress causes people with diabetes to have anywhere from 14% to 20% more cases of anxiety than those without it.  Moreover, it has been reported that up to 40% of the diabetic population show symptoms of anxiety.  A 2013 meta-analysis, including over 12,000 people with diabetes, also found significant associations between diabetes and an increased probability of anxiety disorder or anxiety symptoms.

Chronic stress and anxiety, such as that experienced in diabetes, are often associated with overbreathing.  Overbreathing, or hyperventilation, simply refers to breathing more than your metabolic demands at any given moment, and is often associated with mouth breathing. Consequently, the body gets rid of too much carbon dioxide, which alters the pH of the blood.

This has a similar effect on oxygen and hemoglobin as high blood sugar. 

That is, it tightens the bond between them, reducing the amount of oxygen that can be delivered to the cells and tissues (this is known as the Bohr effect).

Together, we see that the high blood sugar and chronic stress associated with diabetes combine to reduce oxygen availability to the cells and tissues.

When we switch to nose breathing, the volume of each breath is naturally reduced.  This helps normalize carbon dioxide levels and restore blood pH to normal levels, which will improve oxygen delivery.** 

Combining this with the earlier discussion on nitric oxide, we see how this one simple change (nose breathing) helps offset some diabetic complications.

Nose Breathing, Sleep, and Diabetes: The Missing Link to Better Blood Sugar Control

If we breathe nasally during sleep, all of the benefits of nose breathing continue throughout the night. Of all the things nose breathing helps with, this might be the most critical for diabetes.

We have already discussed that diabetes causes chronic stress, which can lead to more rapid breathing.  Nose breathing helps naturally slow down the breath.  This will help you shift from a stressful sympathetic state to a calming parasympathetic state.  This shift is significant for people with diabetes who exhibit less parasympathetic tone at night than non-diabetics.  Thus, nasal breathing at night helps us increase parasympathetic tone and enjoy better sleep.

Receptors in your nose also act to maintain rhythmic breathing during sleep.  This might help explain why nose breathing reduces the risk of obstructive sleep apnea when compared to mouth breathing.  Diabetes is associated with a significantly increased risk of obstructive sleep apnea. So, if nasal breathing at night can help reduce this risk, it could be especially beneficial.

Lastly, we know that inadequate sleep causes insulin resistance.  By getting deeper, more restorative sleep, insulin sensitivity can be improved.  This could potentially lead to better morning blood sugars (that was my experience), setting you up for a better day of glucose control.

The More Subjective but Most Important Benefit of Nasal Breathing for Diabetes

Altogether, nasal breathing increases blood flow, improves tissue oxygenation, and appears to increase an essential form of bioactive nitric oxide that people with diabetes have less of.  It also improves sleep quality by helping us flip to a more calming state and by reducing the incidence of sleep apnea.  This can help improve insulin sensitivity.

When we combine all of these together, nasal breathing's net benefit can be simply stated as:

It gives you more energy. 

And it’s harder to objectively measure, but perhaps the best aspect of increased energy levels is more motivation to take care of your blood sugars. 

Diabetes is an exhausting full-time job that can lead to physical and emotional burnout.  Having more energy and enthusiasm to manage the disease could be the most valuable aspect of something as simple as breathing through your nose.

Footnotes:

*The levels of NO inhaled in the 2019 Cleveland Clinic study were greater than those produced in the nose.  But, the study validated that there is a mechanism by which NO that is introduced into the lungs can be transported throughout the body as SNO-Hb.  Thus, nasal breathing would only act to bring SNO-Hb up to normal physiological levels, whereas mouth breathing would rob the body of this important physiological process.
